Echocardiogram Sample Transcription Report

DATE OF STUDY:  MM/DD/YYYY

INDICATION FOR STUDY:  Chest pain.

STUDY FINDINGS:  Two-dimensional echocardiographic imaging showed normal-sized aortic root. The left atrium was normal. The left ventricle was dilated with ejection fraction of around 50%. The echo was technically limited. The mitral, aortic, and tricuspid valves open well. Color flow and Doppler study demonstrated poor left ventricular compliance suggestive of diastolic dysfunction. There was a mild mitral and mild tricuspid regurgitation.

IMPRESSION:
1.  Dilated left ventricle with ejection fraction of 50%. Technically poor quality echocardiogram. This ejection fraction may not be accurate.
2.  Mild mitral and mild tricuspid regurgitation.
3.  Left ventricular diastolic dysfunction.

Sample #2

DATE OF STUDY:  MM/DD/YYYY

INDICATION FOR STUDY:  Mitral valve disease.

STUDY FINDINGS:  Two-dimensional echocardiographic imaging showed normal-sized aortic root. The left atrium was dilated. The left ventricle showed normal size with good systolic function. Ejection fraction was 55-60%. The mitral valve was thickened. There was restricted movement of mitral leaflets. There was bowing of anterior mitral leaflet suggestive of mitral stenosis. There was also prolapse of the anterior mitral leaflet. Calculated mitral valve area was 0.96. Aortic valve was calcified. There was mild aortic regurgitation. There was also mild tricuspid regurgitation. Right ventricular pressures were 67.2 mmHg.

IMPRESSION:
1.  Rheumatic heart disease with rheumatic mitral valve disease and significant mitral valve stenosis.
2.  Preserved left ventricular function.
3.  Dilated left atrium.
4.  Mild tricuspid and aortic regurgitation.

This study showed that the patient has significant mitral stenosis. Also, there was a mild degree of mitral regurgitation.

Cardiac Stress Test Samples

Sample #3

DATE OF STUDY:  MM/DD/YYYY

INDICATION FOR STUDY:  Chest pain.

STUDY FINDINGS:  Two-dimensional echocardiographic imaging showed normal-sized aortic root. The left atrium was dilated. The left ventricle showed concentric hypertrophy with good systolic function, ejection fraction of 60%. Mitral leaflets were thickened. Aortic valve was calcified. Tricuspid was normal. Color flow and Doppler study demonstrated mild mitral and mild aortic regurgitation.

IMPRESSION:
1.  Concentric left ventricular hypertrophy with good systolic function, ejection fraction of 60%.
2.  Mild mitral and mild aortic regurgitation.

Recommend bacterial endocarditis prophylaxis.
